Humana Military Disease Management Program

The Humana Military Disease Management Program assists beneficiaries in the TRICARE South Region who are not 65 years or older and eligible for Medicare. TMA identifies beneficiaries with certain disease states who are then contacted by a registered nurse. This nurse will work with the beneficiary and his or her physician to identify problems, establish goals, and monitor those goals through regular follow-up care. Humana Military offers disease management programs to those beneficiaries identified by TMA for heart failure, asthma, and diabetes.

Each beneficiary will be able to contact his or her assigned nurse as needed to review treatment plans and to get answers to questions that arise as treatment is administered. The program’s goal is to improve the patient’s symptoms, functional abilities, and overall quality of life. This is achieved by closely monitoring the patient’s treatment program and his or her regular activities.

Warrior Navigation and Assistance Program

Humana Military created the Warrior Navigation and Assistance Program (WNAP) to support ADSMs, National Guard and Reserve members, and their families (and their health care providers) with information and assistance about the TRICARE program, and to help them make a seamless transition through the MHS. WNAP offers person-to-person guidance and access to an advocacy unit that is specially trained on the unique challenges many wounded, ill, or injured warriors face in accessing care. The program is designed to connect warriors and their families with the resources to return them to productive lives and help them over the hurdles they encounter.

Whether the combat veteran is an ADSM, National Guard or Reserve member, or is being medically retired, he or she can be assured that there is guidance available. Returning warriors who have had difficulty accessing sources of support and navigating through the military health care systems, the Veterans Administration health systems, community resources, and the civilian health care sector can find information on these resources and more in the WNAP Portal.
