Disease Management Banner

The goal for the Disease Management Information Library is to provide patient education for specific chronic conditions, directing the patient towards self management of their personal health as well as giving them tips on how to best work with their health care team.  Our intention is to continue to add other chronic illnesses to this library of information.

Heart Failure

Due to the high degree of self-management needed with the heart failure condition, exercise strategies, healthy recipes and material covering related conditions are included as part of the information provided.
    

Diabetes

A dramatic increase in the number of diabetes diagnoses makes this section one of the most active.  Beneficiaries are given healthy recipes, exercise ideas and strategies towards maintaining a healthy attitude while managing their condition.
       

Asthma

Patients diagnosed with asthma often need education on recognizing the triggers that cause asthma episodes and the strategies they can use to avoid them.  Topics of interest include travelling with asthma, sports, work-related asthma and clean home tips.
      

Chronic Obstructive Pulmonary Disease

With COPD, patients are often overwhelmed with concerns about how they can navigate frustrating symptoms, access smoking cessation strategies and develop disease management and coping skills.  These topics and more are covered in the COPD section.
          

Major Depression

Military families often experience unique stresses that can lead to major depression.  Browse through a wealth of helpful education and management information that is intended to help the individual along with his or her friends and family.
     

Anxiety Disorders

Patients diagnosed with anxiety and panic disorders can feel isolated and misunderstood.  The covered topics in this section will not only direct them to find answers and resources to seek help, but also help them understand and manage their condition.
