Am I Eligible?
To be eligible, you must be a National Guard or Reserve member on active duty for more than 30 consecutive days (under federal orders). You are considered an “active duty service member” when on active duty orders. Active duty means full-time duty in the active military service of the United States. Throughout this handbook, when we refer to active duty service members, we are also referring to you. Additionally, your DEERS information must be accurate and up to date. Regardless of your status, if your DEERS information is incorrect or outdated, you may not be considered eligible to enroll. Finally, while on orders, you must live and work more than 50 miles (or approximately an hour’s drive) from the closest MTF.
Do I Live and Work Far Enough from an MTF?
Visit the TPR Web site and type in your residential and work ZIP codes to determine your eligibility, or call Humana Military.
Are There Exceptions to the Eligibility Requirements?
If you live or work within 50 miles of an MTF, you will generally not be eligible for TPR. But you may submit a TPR Determination of Eligibility Enrollment Request form if you believe geographic boundaries cause you to drive more than one hour to an MTF. The request must be directed through your unit commander to the TRICARE Regional Office (TRO) in your area. Visit the Beneficiary Web Enrollment Web Site to submit your request online.

National Guard and Reserve family members are eligible for TPRADFM if their sponsor is activated for more than 30 consecutive days (under federal orders). You are considered an “active duty family member” when your sponsor is on active duty orders. Throughout this handbook, when we refer to active duty family members, we are also referring to you.Additionally, you must reside with your sponsor (within a TPR-designated ZIP code) upon activation or effective date of orders. The sponsor is not required to be eligible for, or be enrolled in, TPR for you to be eligible for TPRADFM. Finally, your DEERS information must be accurate and up to date. Regardless of your status, if your DEERS information is incorrect or outdated, you may not be considered eligible to enroll.
Once enrolled, you may remain in TPRADFM for the entire period of your sponsor’s activation, regardless of any changes in your sponsor’s duty location. However, if you move from the TPRADFM location where you are enrolled or if your sponsor retires, you will lose TPRADFM coverage and will be eligible for TRICARE Standard, TRICARE Extra, or TRICARE Prime (where available).

How Do I Enroll?
Your sponsor can include all eligible family members on his or her TRICARE Prime Enrollment Application and PCM Change Form. However, sponsors are not required to be enrolled in TPR for family members to enroll in TPRADFM. Follow the instructions on the form or the guidance provided if you call Humana Military.
