Mail or fax your completed TRS Request Form along with two month's premium payment to Humana Military within the specified deadline. Initial premiums can be paid by check, money order or cashier's check (payable to Humana Military), or by Visa®/MasterCard®.

Also at the  time of your initial enrollment, you must select an automated monthly payment option for subsequent monthly TRS premiums.  Complete either an Electronic Funds Transfer (EFT) or Recurring Credit Card for Charge form and submit with your TRS Request Form and first two month’s payment.

For those members of the selected reserve who do not have a CAC card, you now can go to any TRICARE Service Center and acquire a DS Logon account.  Once you have a DS Logon, you can use it to purchase TRICARE Reserve Select health care coverage online.
How do I pay my fees electronically?
Note: Effective January 1, 2013, direct billing will no longer be an option.  All accounts must select an automated monthly deduction through either EFT or automatic cred/debit card withdrawal.
