Enrollment and Coverage

       

Eligible beneficiaries losing regular TRICARE or TAMP eligibility must enroll in CHCBP within 60 days following the loss of entitlement to the Military Health System. TRICARE Reserve Select (TRS) members with remaining CHCBP eligibility must enroll in CHCBP within 30 days following the loss of TRS eligibility. Refer to the CHCBP handbook for exceptions.  To enroll, you will be required to submit:
  • A completed CHCBP Enrollment Application form (DD Form 2837). 
               
  • Documentation as requested on the enrollment form, e.g., DD-214-Certificate of Release or Discharge from Active Duty; final divorce decree; DD1173-Uniformed Services ID Card. Additional information and documentation may be required to confirm an applicant's eligibility for CHCBP. 
                
  • A premium payment for the first 90-days of health coverage. 
The premium rates are $933 per quarter for individuals and $1,996 per quarter for families. Humana Military Healthcare Services, Inc. will bill you for subsequent quarterly premiums through your period of eligibility once you are enrolled.  Please call 1-800-444-5445 to pay your enrollment fee.

The program uses existing TRICARE providers and follows most of the rules and procedures of the TRICARE Standard program.

CHCBP eligibility begins immdiately after the loss of regular TRICARE or TAMP eligibility and is limited to either 18 or 36 months. Eligibility periods are:
  • 18 months for separating Service Members and their families. 
                  
  • 36 months for others who are eligible (in some cases, unremarried former spouses may continue coverage beyond 36 months if they meet certain criteria). 
You may not elect the effective date of coverage under CHCBP. For all enrollees, CHCBP coverage must be effective on the day after you lose military benefits.

CHCBP Eligibility for Recalled Reservists/Guard (R/G) recalled for more than 30 days

Criteria Eligibility
Scenario 1:
  • Last active duty/TAMP* status ended within last 18 months
  • Still a member of the R/G
  • Enrolled in TRS
Runs concurrently with TRS eligibility for 18 months
Scenario 2:
  • Last active duty/TAMP* status ended within last 18 months
  • Left the Reserves/Guard after the end of last Active Duty/TAMP status
  • Was enrolled in TRS until the end of R/G status
  • After leaving TRS, apply for CHCBP within 30 days.
Once R/G status ends, you are eligible for any time remaining in the 18 month CHCBP eligibility period that began immediately following the end of the Active Duty/TAMP status. You must enroll in CHCBP within 30 days of end of TRS enrollment.
Scenario 3:
  • Last active duty/TAMP* status ended within the last 18 months
  • Left the Reserve/Guard after the end of the last Active Duty/TAMP status
  • Never enrolled in TRS
  • Applied for CHCBP within 60 days of the end of AD/TAMP status
Eligible for 18 months CHCBP coverage, but since member did not enroll in TRS, must enroll in CHCBP within 60 days of end of Active Duty/TAMP status.
Scenario 4:
  • Last active duty/TAMP* status ended beyond the last 18 months
  • A member of the Reserves/Guard
  • Enrolled in TRS
Not eligible
Scenario 5:
  • Last active duty/TAMP* status ended within the last 18 months
  • A member of the Reserves/Guard
  • Chose not to enroll in TRS
  • Applied for CHCBP within 60 days of end of active duty/TAMP eligibility
Eligible for 18 months coverage, but must enroll in CHCBP within 60 days of end of Active Duty/TAMP status if choosing not to enroll in TRS.
Scenario 6:
  • Last active duty/TAMP* status ended beyond the last 18 months
  • No longer a member of the Reserves/Guard
  • Enrolled in TRS until the end of R/G status
Not eligible
If you are recalled for less than 31 days you are not eligible for the Active Duty Prime benefit, therefore you are not eligible for CHCBP.
*For those not eligible for TAMP benefits (not recalled in support of a contingency operation), any CHCBP eligible begins at the end of your active duty status.  For those eligible for TAMP, any CHCBP eligibility begins at the end of the TAMP status.
