Changing your PCM?
You may submit a request to change your Primary Care Manager (PCM) at any time. All change requests are subject to the Military Treatment Facility (MTF) Commander guidelines. All PCM changes are not effective immediately, so please do not seek services until you receive confirmation of the change.

Are you Relocating?
If you are moving within the South region, you should complete an Enrollment/PCM Change form to change your address and PCM.

If you are relocating outside of the South region, you will need to complete and submit a TRICARE Prime enrollment form in your new region, to transfer your enrollment, update your address and select an Primary Care Manager (PCM)

If you relocate to an area where Prime isn't available, you should disenroll.

Have You Had a Change in Your Personal or Military Information?
It is your responsibility to ensure that the Defense Enrollment Eligibility Reporting System (DEERS) reflects accurate and current information regarding you and your family.

Can I Enroll to my MTF?
 
If you reside within a thirty minute drive time from your nearest MTF and they have space available for your beneficiary category, you will be assigned a PCM at the MTF.

Establish your Monthly Payment Option

Allotment from Retired Pay
  To choose this option, you must complete the Enrollment Fee Allotment Authorization Letter. Once authorized your TRICARE Prime enrollment fee will be deducted from your retirement pay.
New! Pay-by-Phone IVR System
To choose the new IVR system
Call: 1-800-444-5445. Pay-by-Phone allows you to make a payment over the phone 24 hours a day, seven days per week. It also provides information and allows you to request forms to set up future automatic monthly payments.
Electronic Funds Transfer
  To choose this option, you must complete the EFT Payment Authorization form. Once authorized, your TRICARE Prime enrollment fee will be deducted automatically from your bank account (checking or savings) each month.
