|
If you are called again to active duty for more than 30 days while on TRS, you will not be covered by TRS during active duty and you will not pay monthly TRS premiums during that time. When you return from active duty, you should contact Humana Military for information about whether or not your TRS coverage will continue. It is important that you do this so you’re aware of your health care status and premium payment responsibilities, because in some cases, Tier 1 coverage automatically continues and premium payments automatically resume.
Note: If you and your spouse are both National Guard or Reserve members and you are the sponsor for TRS coverage and your spouse is called to active duty, you will have to contact your Service personnel office to ensure that your TRS coverage and monthly premium payments are stopped. You will need to visit your Service personnel office when your spouse returns from active duty to reactivate coverage.

When TRICARE Reserve Select Coverage Ends |
|
Your TRS coverage terminates when you reach the end date of your coverage, you no longer meet the required qualifications, or when you send a termination request to your regional contractor (for Tier 1 only). Coverage also will end if you fail to make a monthly premium payment on time, but you will still be responsible for paying any overdue amounts. When your TRS coverage is terminated for any reason, your family members’ coverage automatically ends as well. |

Certificate of Creditable Coverage |
|
When your TRS coverage ends, you will automatically receive a certificate of creditable coverage. The certificate of creditable coverage is a document that serves as evidence of prior health care coverage under TRICARE so that you cannot be excluded from a new health plan for pre-existing conditions.
Certificates automatically reflect the most recent period of continuous coverage under TRICARE. Certificates issued upon request of a beneficiary reflect each period of continuous coverage under TRICARE that ended within the 24 months prior to the date of loss of eligibility. Each certificate identifies the name of the sponsor or family member for whom it is issued, the dates TRICARE coverage began and ended, and the certificate issue date.
The Defense Manpower Data Center Support Office (DSO) will issue a certificate of creditable coverage to sponsors and family members automatically upon loss of eligibility.
Send your written requests for a certificate of creditable coverage to the DSO at:

The request must include:

- Sponsor’s name and Social Security number
- Name of person for whom the certificate is requested
- Reason for the request
- Name and address to whom and where the certificate should be sent
- Requester’s signature

You cannot request a certificate by phone. If there is an urgent need for a certificate of creditable coverage, fax your request to the DSO at 1-831-655-8317 and/or request that DSO fax the certificate to a particular number.

Continued Health Care Benefit Program |
|
Once your eligibility under TRS ends, you may be able to apply for temporary, transitional medical coverage under the Continued Health Care Benefit Program (CHCBP). CHCBP is a premium-based health care program and is similar to, but not part of TRICARE. If you qualify, you must enroll yourself and your eligible family members in CHCBP within 30 days after termination of TRS coverage. Benefits under CHCBP are virtually the same as those under TRS. To find out if you are eligible for CHCBP, contact Humana Military Healthcare Services, Inc., at 1-800-444-5445 or visit the CHCBP portal. |

When a TRICARE Reserve Select Sponsor Dies |
|
If the Selected Reserve member (National Guard or Reserve sponsor) dies while in a period of TRS coverage, his or her family members may be eligible to purchase continued coverage. They should contact their Service personnel office for more information.
This is separate from survivor coverage if a sponsor dies while activated. You may contact Humana Military for more information if you find yourself in this situation.
