You and each enrolled family member will receive a TPR or TPRADFM enrollment card, which shows the effective dates of your enrollment. Included with the card is a welcome letter identifying your PCM's name and telephone number, if assigned. Write your PCM’s name and telephone number on your card. TRICARE network providers may require you to show the enrollment card as well as your uniformed services identification (ID) or Common Access Card (CAC) at the time of service.
The TPR and TPRADFM enrollment cards do not verify your eligibility for TRICARE. Only your DEERS record can verify eligibility.
Enrollment in TPR and TPRADFM is continuous—you do not have to re-enroll every year to maintain coverage. However, certain events will cause you to be disenrolled.
Sponsor Status Change
Any change in the sponsor’s status (e.g., retirement or National Guard and Reserve member deactivation) will cause automatic disenrollment from TPR/TPRADFM. In some instances, you may not be able to re-enroll in TPR/TPRADFM. For example, if you were enrolled in TPR and you retire from active duty, the TPR option is no longer available to you. To continue TRICARE Prime coverage, you’ll need to move to an area where TRICARE Prime is offered and enroll. If not, your coverage will continue under TRICARE Standard and TRICARE Extra.
Note: In some cases, you may be able to waive your TRICARE Prime access standards and enroll in TRICARE Prime, even if you reside outside of a TRICARE Prime service area. Contact Humana Military for details.
Voluntary Disenrollment
Active duty sponsors must be enrolled in TPR in remote locations. Family members, however, may decide to use TRICARE Standard and TRICARE Extra. If family members choose to disenroll from TPRADFM before the annual enrollment renewal date, they may be subject to a 12-month lockout,* during which they will not be permitted to re-enroll in TPRADFM. You must contact Humana Military to initiate a voluntary disenrollment.
* The lockout provision does not apply to active duty family members of sponsors grade E-1 through E-4.
Important Note for Family Members: If your sponsor is deployed, you may remain enrolled during his or her deployment. However, if you move from your current TPR zip code area, you must disenroll from TPRAFDFM, and you may not re-enroll even if you move to another designated TPR zip code area.
Note: You are able to move within the same zip code area and remain enrolled in TPRADFM. In these situations, you can enroll in TRICARE Prime, if available, or you are covered by TRICARE Standard and TRICARE Extra.
