Q:  Are flu shots a TRICARE covered benefit?
 
Flu vaccines are a TRICARE covered benefit following the CDC guidelines.
  
Q:  Is the vaccine that may help to prevent the Human Papillomavirus (HPV) a covered TRICARE benefit?
 
Following a Centers for Disease Control and Prevention (CDC) recommendation, TRICARE recently made Gardisil®, the vaccine that may help to prevent the Human Papillomavirus (HPV) and ultimately cervical cancer, available to its beneficiaries as a covered benefit.
   

Q:  What immunizations do the schools require for children and at what age and frequency?
 
Check with your local school district because requirements are different throughout the country. Locate your states schools requirements.
 
Q:  Where can I find the current list of preventive service immunizations?
 
TRICARE covers CDC approved immunizations and vaccines according to age and frequency guidelines from birth through adulthood. Check with your Primary Care doctor to confirm when you or your child is due for an immunization or vaccine.
   
Q:  How do I submit a claim when I have paid the out-of-pocket charges?  What information is needed?
  
To file a claim, obtain and fill out a DD Form 2642  Patient’s Request for Medical Payment. 
  • When filing a claim, attach a readable copy of the provider’s bill to the claim form, making sure it contains the following:
    • Sponsor’s Social Security number (SSN) (Eligible former spouses should use their social security number.)
    • Provider’s name and address (If more than one provider’s name is on the bill, circle the name of the person who treated you.)
    • Date and place of service
    • Description of the service
    • Charge for the service
    • Diagnosis (If the diagnosis is not on the bill, be sure to complete block 8a on the form. For Immunizations, "preventive service" can be entered.)

Be sure to complete all 12 blocks of the form correctly and sign it.

Be sure to keep a copy of the paperwork for your records.  If you live in the South region and have a question regarding claims processing, you may call Humana Military's claims processor, PGBA, at 1-800-403-3950.  Check the status of your claim at the PGBA Web site.
