Following a Centers for Disease Control and Prevention (CDC) recommendation, TRICARE recently made Gardisil®, the vaccine that may help to prevent the Human Papillomavirus (HPV) and ultimately cervical cancer, available to its beneficiaries as a covered benefit.

Q: What immunizations do the schools require for children and at what age and frequency?
Check with your local school district because requirements are different throughout the country. Locate your states schools requirements.

Q: Where can I find the current list of preventive service immunizations?
TRICARE covers CDC approved immunizations and vaccines according to age and frequency guidelines from birth through adulthood. Check with your Primary Care doctor to confirm when you or your child is due for an immunization or vaccine.

Q: How do I submit a claim when I have paid the out-of-pocket charges? What information is needed?
To file a claim, obtain and fill out a DD Form 2642 Patient’s Request for Medical Payment. |

Be sure to complete all 12 blocks of the form correctly and sign it.
Send your claims to the claims processor for the region in which you live. If you live in the South Region send your claims to:
